I accidentally deleted a form I need

Is the issue still persists? Try using a different internet browser or refreshing your internet browser. As long as you did not Purge the form, you can restore it back.

May I know what is the complete form name so we can further check it?

I found the form in your Trash folder. To restore it, select the form and click the Restore button.

I checked your account and you have 5 active forms. Keep in mind that the Free account can only hold up to 5 active forms. You need to make room for the deleted form in order to restore it properly.
