Please update your steps as Step 3 is omitted and it takes you directly to Step 4 and if you don't like the card look you have to go into Form Setting > Show more options > scroll down and look for Form Layout and change the option.

Actually, the guide is correct, it's just, some of our users can choose initially Classic or Cards layout (which you can switch from settings as you noted as well).
