We can create the above form by using simple check boxes and text fields as shown below. I hope based on my understanding of what you need the form below will work well for you.

Thank you Trevon, however, this is written information needed to make a tag for each flag. I have check boxes for those questions that are standard and can be answered with a check off.
In Honor of or in Memory of: (Please circle one) (I have a check off box for this answer)
The rest of this has to be filled in! How can I make a write in for each questions.

If I get you correctly you want to have users fill in or type  the data in the format provided but in one element. You can achieve this by using the Long Text element in your form.

    Kindly see demo form below:

    https://form.jotform.com/73613570139961

Kindly feel free to clone and use the form.

If the above is not the case would you kindly share a pictorial illustration of the functionality you need for us to give you a conclusive solution.

Below is how you can share your illustration with Jotform:

    Thank you Trevon!
I figured out what to do.
However, the idea of people going into my PayPal account is not ideal for security. I thought I could put a link in the form to help them pay for their order would be best.
    What do you suggest?
