Why is my login suspended, and have my forms been disabled ??

Your account janputcuyps is suspended because of the form Carya Group Nieuwe Werknemer INCADEA.DMS.
Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
In order to activate your account, it is required to agree to our terms. We will also have to delete mentioned fields from your form.

Those fields are not for collecting existing credentials. They are used to ask new clients for the new credentials they wish to use. Sort of as a signup page.
We are currently looking at a selection of 'form providers', like Jotform, Cognitoforms and others, to use for our customer portal website. Before we signup for a paid service we would like to be sure our forms will not be disabled without notice.

Most online form builders, including JotForm, are being used for identity theft. We go to great lengths to prevent this. That is why our Terms mention that a field named password is considered a Phishing activity.
Can you please explain to us the purposes of the form.

The purpose of the form is to receive information for the registration of new users for our software platform. New users are allowed to choose their own user ID and password.
We use this information to create the users in ERP software.
The form will be embedded in an iframe on a secure page in our Zendesk.
