    My forms are not working and when I tried to login it says I'm suspended

    Your account (michelegracetravel) is currently suspended. The form (Credit Card Authorization) is the one that triggered the suspension.

    Please take note that collecting credit card details using regular text fields are not allowed here in JotForm. Please check our terms of use.

    We need to delete the guilty form or guilty fields so that we can reactivate your account. If that is okay with you, please let us know.

    If you need to collect payments from your users, please use one of our payment processors. Here's a guide: Setting-Up-Your-First-Order-Form. You may also capture the card details and charge the user at a later date. Here's a guide: How-to-Enable-Payment-Authorization.

    Thank you for you response.  Please delete the offending fields and I will use one of the payment authorization options you suggest.   Please let me know how to get back into my account.

    I deleted credit card fields from your form and I activated your account. 

    Please do not create similar forms. If you want to collect payments, please use one of our payment fields. For more details check the guides that my colleague provided you.
