Our form on http://dailyadrenaline.org/ (under the "Give Once" button) or direct link https://form.jotform.us/71803933230147 has a Single Choice Properties button for people to mark "Yes" if they would like their donation to remain anonymous.
For some reason when a form is submitted no info comes through for the Anonymous single choice property. The field doesn't show up in an excel export on under view submissions. Do we have something set up improperly?

It wasn't in your email notification template so I added it. Please check it now.
It is not displayed in your form submissions page because they were empty. Empty fields are hidden in the submissions page by default but you can show them.

Hi there, thanks for the response on this. I'm still having an issue though. When the "yes" radio button under anonymous is checked on a submission it still comes through as "-" within Jotform. I just ran a new test charge for $1 and made sure to check "yes" under anonymous.

It's weird - if the "yes" is checked when you hit submit it unchecks the yes. A donation amount is not required to submit so if you wanted to test with just a name and fake email and check anonymous you could.

I did a test submission in your form and the Anonymous field was showing fine, here is the email response:

So it's weird, if you do not include a donation amount then the anonymous radio button works fine. If you enter a donation amount and the necessary credit card info it doesn't record the Anonymous radio button. I recorded to separate videos so you could see the issue.

I was able to reproduce the problem. I have now raised this to our developers for further investigation. We will let you know once this is fixed.

I figured out the problem after analyzing the full source code of your form. There's a calculation script that is stuck in your form source code which shouldn't be there. I will add this in the ticket description.
There's also a "Do It Yourself" fix. You can simply add a new radio button field with the same option in your form then delete the old one. This should fix it. Please be reminded that deleting a field also delete its associated data so please download your form submissions first for your backup copy before deleting a field.
