No matter what i do I can't get my form to work right!!!!

    Set conditions are not red correctly

    HTML code does not match what is in Jot Form

    Jot form does not read the code correctly

    What am I doing wrong???

    I checked your form and I'm not sure to what you're referring to when it comes to conditions, I noticed that you have two conditions for "complaint" option:

    Which are showing the same fields so you should check that.

    Regarding the HTML, what do you mean "JotForm doesn't read the code properly"?
