Why my account was suspended?

    Hello, 

    My account was suspended and I gave the authorization to clear off all unacceptable fields. Would it be possible for my account to be reactivated?

    I couldn't find any other thread related to your account and suspension.

    Your account was suspended because of this form: http://www.jotform.us/form/73185698672170 and the reason is that you were collecting passport information which is considered as sensitive data and it triggers your account as a possible fisher.

    You have to provide us information about the purpose of collecting such information so we can re-consider re-activating your account. Otherwise, you have to remove those fields.

    Thank you for your response. Yes, I'm responsible for booking flights and getting business visa's for my band (a group of musicians) so I was trying to find a more efficient way of collecting the information so that I wouldn't have to keep digging through emails. I do understand that the information is sensitive and I am more than willing to have it removed from the forms so that I can access my account.
