I am receiving an error message indicating the account was suspended

    I have been utilizing forms for the last few months. However, when I now login, I am receiving an error message indicating the account was suspended. I am not sure why that would be.

    Your account got suspended because the form (http://www.jotform.us/form/72689051684164) is asking for credit card information using the Basic fields and this violates our terms of use.

    If you want to collect payment, please use our payment processor app. Here's sample guide: https://www.jotform.com/help/391-How-to-Integrate-Form-with-Paypal-Standard-

    We need to delete the guilty fields so we can re-activate your account immediately. If that is okay with you, please let us know.

    Thank you for your response. I apologize for the error. I was not aware of
    the terms of use for the form in question. You may delete the form in its
    entirety and I will review the link you provided. Thank you.
