Acknowledgement form

Looking to embed a form on my real estate Tenant Verification website.

I want to ask site visitor for their name, company name, date, and most importantly a signature. I saw the feature you have where the visitor draws their signature and I really like that. The page would be displaying an "FCRA Acknowledgement" pdf. The visitor is basically acknowledging that my we are going to perform a background check on them to approve them.

After this is completed, I want an email to be generated and sent to both myself and the site visitor containing all of this information. Also would like the information to be stored in a database somewhere.

Can someone please contact me and let me know if this is possible, what package i would need, and the costs associated.

That is possible Alex, for signature field that is drawn. You can use our Signature widgets.

You can attach a PDF into the form using the PDF Embedder widget and use the "Terms and Condition" widget for acknowledgement/agreement.

This is possible by setting up Email notification which will notify you if someone has sent a submission, and an Autoresponder notification for your site visitor/customer.
