The result is not the same as the form template when printing answers

I used the free template:

when I print de answer I see that the information is not the same like de template, 

what is the problem?

My understanding is that you want to print the form with the user's answers.

You can insert a "Print" button in the form using the Form Builder and then go to the Submissions page and click the "Edit" button. After that, you just need to click the "Print" button inside the form.
