Hi,
I have Create a form in JOTFORM and I used the widget checkbox in Dropdown, when I use the preview option to see my form every item appears perfectly but when I have downloaded the code source to my local workspace and open the form in any type of browser the widget wasn't visible, I send you the link to my form and a capture of the result. Thank you

Apologies for the inconvenience.
Are you viewing the form URL directly or is it embedded to your site and you are viewing it from your site URL? I've tried viewing your form URL and was able to see the Widget Checkbox.
Kindly try to clear your browser's cache and see if that helps. Let me know how it goes.

Hi,
Thank you for your answer, I am viewing the form URL directly but in local, and I found a solution. Actually I had to change the source file of the widget in the source code like this:

I'm glad you figured it out. You mentioned "I am viewing the form URL directly but in local" so I'm assuming you grabbed the full source code of the form and you're viewing it through localhost.
That might be the reason why the widget isn't loading and why your solution worked. Viewing the form directly through any of the links below should display the widget without issues:
