When trying to use my form I get an error. I did not have this problem before I
upgraded to a paid plan.
I have gotten this error on 2 different computers in 2 different countries.

You seem to have the "Unique Submission" feature enabled. When you disabled the feature your form will be work as you want.
You can see how can do it from the following guide.
1. SETTINGS > FORM SETTINGS
When you come to the Form Settings page, please click the 'SHOW MORE OPTIONS' button.
2. UNIQUE SUBMISSION
You need to change the option from 'Check cookies only' to 'No Check'
