Why does the form not work?

Hello. The submission form I have created and embedded on my conference website does not work sometimes. I have received many e-mails from the participants that they can not open / submit the form. What do you think can be the reason for that? Thank you for your help. 

Hello @burcinky - We'll only know the reason why it's not working once we test it out. Can you share with us the URL of your page where the form was embedded? We'd also like to know if it's okay for us to send test submissions once we have the URL.
