When our clients click the "review and submit" button on our form, it goes blank. They don't get a chance to review and the form is not submitted. It's causing great frustration.

I see the problem the review and submit screen never appears. It seems to hang at this stage. And the problem affects both your direct link and your embedded form. It also affects the clone that I created for testing.
So I have escalated this issue to Level 2 support. They will investigate the problem and post a response on this thread.

I have made required changes for review card. The problem should be solved now.
