Why my conditional logic is not working as it should?

    Hi There

    I'm having trouble with the form below and have tried several ways around it to no avail.

    My first question decides which set of questions are asked (using "Show" conditions).

    If you select the "Equipment Booking" option it shows all fields and will not Hide the fields I have requested based on certain answers (for "Centre" and "Frequency of Booking" questions.)

    Eg If under "Centre" you select "Wallington" it should Hide the "Room Numbers"
    However, if you Select "Sutton" and then change it to "Wallington", the "Room Numbers" remain visible.

    If you select "One Off" under "Frequency of Booking" it does not Hide the "Date of Last Session" question.

    Any help would be very much appreciated. I know I could create 2 seperate forms but i would rather not. I've tried using page breaks but they have created other problems.
