How to create a form with approvers?

What I need to happen at this point is have the Access form do what the other form I have do but there can be many different selections of who to send an email to so here is an example: 

The end user fills out the form stating that this person needs access to both of these applications selected but the two applications have different approvers (Jon and Tom) so I created notification emails to go to the person depending on which box is checked. Right now I have all of the emails going to me until this is corrected so you will see my email in each of them. Once the user submits this they each should get an email stating that they need to approve their section so as an example Toms email should take him to a different section than Jons email takes him to and Tom should not be able to see what Jon is approving so Tom should not see the check mark next to Yardi, only the check mark next to PDI Wholesale. Right now they both get an email and they can both see everything that is checked. Unfortunately, we can't randomly send emails from two selections( Jon or Tom). The user must select one which the emails will go.  We can add a skip a page condition,when the user selects from  Jon or Tom and it will show different pages.
