Why is my account suspended?

    Your account (ARShopzon) is currently suspended. Our phishing detector tool automatically suspended your account.

    This form (Business Associate Application Form) triggered the suspension. I checked it and I can see that it has a field that is asking for IFSC code or Bank code.


    Please take note that collecting sensitive information is not allowed here in JotForm. I suggest you check our term of use.

    We need to delete this guilty field so that we can reactivate your account.
