I cant log back into the account that I made, I have tried to change the password by clicking that I had forgotten my password. If I try and make a new account it says the account name is taken.
If you can let me know what I should do it would be appreciated.

Your account was auto suspended by our anti-phishing system. JotForm has an automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities.
I have checked your forms, and I can confirm that they are not being used for any phishing activities. Your forms are whitelisted and your account is activated.
