Header image is not being displayed in form.

Hi, have added a picture on the form which i can see in build form but not when published or when I send around the link. Please advise on how to correct.

I'm sorry for the trouble caused to you.

The issue was caused because of the uploaded file name. I have fixed this issue in your form by uploading it again. Please test your form and see if the image is being displayed correctly. Please check the screenshot below:

Your image file name has & and additional "." which was causing this issue. 

I downloaded the file and removed the & from the file name and uploaded it again. That seems to have solved this problem. I also replaced space with "_" in the file name but that is not a requirement.
