How to populate fields based on dropdown selection?

    Hi,


    I work for a school district and am trying to create an online form for our employees to fill out during open enrollment to make their health and welfare selections.  What I'd like is for the employee to pick their name from a pull down list, enter their employee number and have the form populate with their current year picks.  Then I'd like them to be able to select the new year selections and have the form calculate their new cost. 


    I'd also like them to be able to submit the form and have the results entered into a google spreadsheet if possible.

    This is possible.

    What I'd like is for the employee to pick their name from a pull down list, enter their employee number and have the form populate with their current year picks.

    You can use Dropdown field element for your employee list and add a Update/Calculate field condition to pre-populate/insert data to the other field elements based on the selected employee.

    Then I'd like them to be able to select the new year selections and have the form calculate their new cost. 

    This can be done using the Update/Calculate field condition too. Or use Form Calculation widget to do the calculation.

    I'd also like them to be able to submit the form and have the results entered into a google spreadsheet if possible.

    So for the first part, I have a pull down list of employee names figured out.  What I'm not sure is how to pre-populate/insert data to the other field elements based on the selected employee.  Where does this pre-populated information sit?  I have about 200 employees and a spreadsheet that shows the current plans they are on.  How do I get this to read that spreadsheet data?

    You can use the Spreadsheet to Form widget in this case. On this test form https://form.jotformpro.com/71500914392957, you can simple input a number which corresponds to the number column where the information will be pulled from.
