Hello, please accept my apologies and I hope that we can help you resolve this issue. This seems to be related to the Android's Default Browser with reading your form from your website.

Have you tried going to it with an Android related third-party mobile browser? I tried this on my iphone and it is working fine in both Safari and Skyfire's browsers just to test it out on a different type of smartphone.

I did some testing and it appears to occur mainly because the form is somewhat lengthy. You may want to try embedding the form within an iFrame on your website to see if that helps correct any issues on Android.

If that does not work, you may want to consider using a Page Break via your form tools which will allow you to separate the form into 2 or more sections thus shortening the page's loading and resources which can sometimes timeout and not work properly due to data limitations on mobile browsers.

I hope this can help but if not please let us know so that we can help you more.
