why would that happen?

I reviewed your form email settings but I did not find anything unusual that will caused the repeated emails.
What I did found however is that there were repeated multiple submissions from the same submission using the EDIT submission.
Because the Send on Edit option is enabled on the email, submitting the edited submission will trigger the email alert every time.
