I get an error message that login is suspended.  I have reset the password and I get the same message.  How do I unsuspend the login?

You are asking for social security numbers in the form called "Goodwill 2018 Employee Benefits Open Enrollment Online Application", that is not allowed: https://www.jotform.com/help/8-Terms-of-Use

"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

The only way to allow collecting some sensitive information such as SSN and drivers license is by adding a signature field, and encrypting your form: https://www.jotform.com/help/344-Encrypted-Forms-and-How-to-Use-Them

"Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information."

So, if you agree to use best security practices, or have the SSN fields removed from your form, we can reactivate your account.

Thanks for your understanding, we have reactivated your account. No, this is not new, since our system detector reviews thousands of forms everyday, it could be that it failed to detect yours earlier.

Open a new thread if you need anything else, we will be glad to assist you.

We agree to use best security practices.  I am going over the how to use encrypted forms now.  I am confused as we use jotform for benefits enrollment.  We have been using it for this over three years with no problems is this something new that was put into place?
