I received support from your tea, implemented suggestions and my conditions worked!
However, I noticed when I came back from the weekend, my updated conditions were not there anymore. They had somehow reverted back to my old conditions and nothing was working correctly.
I've since updated conditions again and everything is working fine but I was curious as to why this might have happened. Is there a way to lock or save these conditions so they can’t be altered with?
Also, does JotForm do some sort of sweep occasionally that might explain this?

That's odd, we would not revert anything in your form. Have you, or someone else, reverted the form using the Revision History? https://www.jotform.com/help/294-How-to-View-Form-Revision-History
