Creating Form Template: Do I use the "CLONE" and duplicate it and then change it and retitle it as needed?

Once we have a design settled on this will be used site wide. Do I use the "CLONE" and duplicate it and then change it and retitle it as needed?

Right now we will need to make 30 different forms but using the same theme throughout.
