Some fields on my form didnt show replies from user. I tried to use the form as a client but the fields are blank when i receive it on my inbox.

Rebuilding your form notifications would solved your problem. I have cloned your form and see image below after I modify your form notifications fields, the "Answer" fields.
Please let us know if the problem still exist after doing above solutions.
