Quiz Form App: Results are not being displayed

the form does not seem to be working as expected

the top few answers are registered, but the rest are not

pls advise

Based on the screenshot you provided, it looks like you are referring to a different form in your account. 

I can only assume that you are using the Custom Results/Email Page of the Quiz Form App. And the issue with the result that you are getting might be a result of unmatched field question names. How-to-Find-Field-IDs-and-Names

To fix the issue, you can click the "Reset to Default" button in the Quiz Form App as shown in the screenshot below.
