Based on the screenshot you provided, it looks like you are referring to a different form in your account.
I can only assume that you are using the Custom Results/Email Page of the Quiz Form App. And the issue with the result that you are getting might be a result of unmatched field question names. How-to-Find-Field-IDs-and-Names
To fix the issue, you can click the "Reset to Default" button in the Quiz Form App as shown in the screenshot below.
