What field is needed to use the Submission Verifier app?

hi there,

I’m trying to add the Submission Verifier app to my form but after signing in, clicking on “Launch app” and select the form, I get a error message saying “This form does not contain a valid question. Please select another form.”

what question am I supposed to add? There’s no step by step tutorial, no FAQ, no explanation, no nothing.

please help!!!

The app requires any of the following fields:

🔘 Hidden Box (recommended)

🔘 Text Box

🔘 Text Area

🔘 Drop Down

🔘 Radio Button
