Old Layout: Why is the header image not showing up?

    Why doesn't my photo/image show up on my final form?  When I pass along the link and folks click on the link, the image in the header section does not show up.

    I have checked your form and the image link seems to be broken, please try re-uploading the image, if possible, remove the spaces from the image's name in order to avoid conflicts due to this. 

    If the problem persists, let us know.
