How to customise feedback forms to fit into our own?

We have our own training and would like to do an online feedback forms and in doing so, the attendee will get a copy of the training materials.

First of all, please sign up to JotForm to create a free account. Then, you can browse the templates and choose the best fit for your feedback form. You can edit the template you choose, you can add different types of questions or delete the unwanted ones. All you need to do is to use the form builder. 

Also, if you ask for responder's email address, you can edit the autoresponder settings (Settings > Emails > Autoresponder) to send email to the form responders. You can determine the email content and attach "copy of the training materials"
