Asana Integration is limited only by a team.

Your form is limited by the team, if we are a small company that needs 1 form service that can be used by all members there is no need for teams. However, your form cannot continue without a team selected.

You're right and I was able to reproduce the problem on my personal account. I can only set up to the Workspace field but can't proceed with mapping the fields.

I will escalate this to our developers and see if there's anything they can do for this.
