I cant reset my password and get into your site where I have a LIVE survey with an incorrect field.
The phone number field is saying it needs an email address.
THIS IS MEGA URGENT, PLEASE HELP!

I checked your email address and there is not any JotForm account related, I checked the form you provided as well and it was created under a guest account, please note that those accounts are stored on the browsers data and do not have a password.
Please try creating an account using the same email address as the guest account so the forms are tied automatically, the address is F*****e*****s@r***i****r.jotform.com.
