I have had many problems with people not being able to complete the form, and then not being able to complete payment once they do have the form completed. On the form I built, I had a spot for name, email and phone, but once I enabled the form, those spots do not come up. Also I had to put photos in several times for them to be seen.

The full name field in your form is conditionally shown based on a field that has been deleted:
Deleting that condition would allow for the full name field to be shown. The same goes for your phone number field.
It looks like the last few submissions did go through with payment. Let us know if that issue persists and we will look into it further.
