Why your embed code can not work on FB app or on mobile ?

I installed as your step sothat I have new tab on FB on mobile .But it can not work

why ? what happened ? help me to explain in this problem ,thanks

If you embedded your form on a Facebook tab then it will not be shown on mobile, it's not currently supported by Facebook. 

I would recommend you to alternatively embed your form following this guide, it generates a link to be used on mobile: https://www.jotform.com/help/82-Adding-a-form-to-Facebook-Custom-iFrame-App-

As already mentioned as a FAQ #1 in the guide Adding-a-form-to-Facebook-Custom-iFrame-App-, the page tab apps are not supported on mobile devices by Facebook's Platform. However, the static Iframe Tab App provides direct URL to your page tab that is viewable on mobile devices. To get your page direct URL, simply edit your Facebook tab and copy the link you see on the Tab URL box.
