Did the way your forms work change? If I clone on of my previous form it display differently now.

Yes we recently introduced a new form layout. It puts one question on one page. 

The old form layout uses all questions on one page.

In your forms settings it is possible to switch individual forms.

In your profile settings you can switch the default type for your account.
