How do I remove the template name?

    When I post the registration form - instead of saying the name of our form, it is identified by the template name. 

    I need to remove this (see picture - it says Professional Development Workshop Registration Form)

    and replace it with the actual name of the registration form (Temiskaming Multiple Births - 2018 Registration Form)

    but don't know how.  Help!
