Why is my form autosuspended?

Hello, I have been working on a form to serve as a voting ballot for our awards show. I was logged out and tried to log back in again, only to see my account is "Autosuspended." My username is phwa_voting . Please help!!!!!

I see my colleague Adrian has already reactivated your account. You should be able to log in and work away.
