Check boxes on booking form

I  want to create a booking form similar this form shown here: http://www.newbornphotographyessex.co.uk/machform/view.php?id=41429 Most of it is simple enough to do, but how do I create a section which has text and its own checkbox - as shown on the above form by the signature box. 

Any advice would be appreciated.

You can add a Multiple Choice or Single Choice form element to your form to achieve that.

You can also make it required in the field properties if needed.
