How to restore my deleted form?

Go to your myforms, on the left side bar you can find the text Trash click it and the Trash  window will open.

This is the window that holds all your deleted forms, select the form you have deleted and an option above will be visible and then click Restore.

Now you form has been restored.

Do note that, Purged forms can't be restored.
