Emails: Not showing all fields

It looks like this:

I think you are referring to the notification email.

In the email body you are missing the "Surgery case" label and corresponding variable.

Also the {input_a_certify} variable is missing a label.

If you are missing data on your email. It could be because your clients might supply blank answers. You have the "hide empty fields" option enabled. So you should disable this.

A simple solution for you is to delete this notification and add a new one. It will automatically include all fields on your form. Then just manually disable "hide empty fields" if you wish.
