Request to send subscription invoices via email

Hi,

I manage the Jotform USTA Officiating Department account. I am looking for an invoice for November, December, and January for our account. Our account name would be either USTA or United States Tennis Association. If you can provide this to me via email that would be great. My address is dennis@usta.com
