How I add more one form in may fanpage?

Hi!
I'm using Jot Form in my Super TopHaus fanpage (facebook)
I already have one form, but now I need add one more. But when I need choose the fan page, SuperTopHaus doesen't  Show.

Why this happen?

Eu estou usando o Jot Form na minha fanpage SuperTopHaus. já tenho um formulário mas agora preciso adicionar mais um. Já criei o formulário mas a opção desta fanpage não aparece mais para mim quando devo escolher em qual fanpage devo inserir o formulário.

Por que isso acontece?

As for now, you can only add one form to your Facebook page.
