I am having problems creating the Quiz form from an Excel file

Thanks for your help....I'm trying to create a quiz in jotform.  I know you have a platform for quizzes but first I have to create the form.  This is where I'm having problems.  I've got the quiz set up in Excel right now.  

I don't exactly get the excel, but are VATA, PITTA, KAPHA your questions? I would suggest creating your form first then explore the fields we have. For quizzes, it is the Single Choice, Multiple Choice, and the Dropdown fields that you are going to use for selections. 

If you can explain how the excel works, we'd be happy to create a demo form for you.
