After you hit submit it gets stuck on please wait and never submits

My form doesn't seem to be working properly on my website- After you hit submit it gets stuck on please wait and never submits. Can you please see what might be wrong.

I also don't like how it doesn't fully display the form and you have to scroll down to answer questions.

Can you please try first re-embedding the form in our website using instead its iframe publish code.

Check/test if using the iframe publish code resolve the issue already. Let us know if still not.
