The email submission I'm getting notifying me of artist application is always missing fields? When a artist applies I cant even seem to get in the subject line the artist name and location??
Can you please advise why pdf attachment is not providing all info (i.e. name, images are always missing)Page URL:

I have checked your form and the email notification seems to be blank, but you may edit the email notification to include on the subject any submitted info, this guide will help you editing the email template: https://www.jotform.com/help/430-How-to-Edit-the-Email-Template-for-Notifications-and-Autoresponders#_
Also, I can see from the email logs that you're including the link to the PDF report, I would recommend you to reset the PDF layout in order to show all the fields, this guide will help you getting into the customization page where you can reset the report: https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report
This guide will also help you including all the static text from your form in your PDF report: https://www.jotform.com/help/305-How-to-Show-Headers-and-Text-in-the-PDF
If you keep experiencing issues, I would recommend you to delete the email and add it once again:

I have reset the pdf still some fields missing??? why is this happening. I want all fields to show but bank details missing same as travel fee???

The issue continues? Now I’m not even get images and missing the field travel cost field

I did not find any field on you form labeled as "field travel cost" or similar, I have also checked your PDF report and it seems like you have customized it and removed some of the fields from it, on your form there are about 28 fields, but on your PDF report only about 15 fields are included.
Please let us know the labels on your form of those fields missing on the PDF report, we will be glad to further investigate this.
