    How can I have the selections from my checklist consolidated?

    After a user checks their selections, I would then like their selections to consolidate in a new window. Next to each selection, I would like a rating scale that they can rate the importance of their selection. Thanks!

    Unfortunately, we do not have any tool that can consolidate the users selection and use that data through the form submission process.

    I would like to suggest trying the following work around with conditional rating scale fields that are being shown when some option is checked.

    Example:

    I have added some rating tools to the cloned form.

    Then, set up Show conditions to show mentioned rating fields when the users click on the options.

    It is possible to do the same for every question.
