How can I make a single form handled by two or more people/users?

Hi,

I need to create a form in such a way that after submitting it, it should go to a user (whose email id is mentioned in form itself) with some new editable fields along with the original fields filled by first user (which should be non-editable for second user). 

Can we create such form by any way?
