We had noticed that we had not been getting any forms from our website. When you logon to our JotForm account there are no forms showing up .

It seems that the form has not been submitted recently. When I checked the submission data, the most recent submission was received on 2017-10-20.
To make sure that the form is working, I have submitted a test entry and it went through without any issues.
