I received notification that my account with JotForm has been suspended.

We apologize for the inconvinience caused. The form was mistakenly autosuspended by the phishing system, probably due to combination of elements in the form. But I checked your form, and found nothing suspicious about its purpose.
I have now removed the autosuspended status and whitelisted the said form. I check your website and see that your form is online again.
